How to Apply  
   
Ensure you meet the
eligibility requirements
  • Hold an Illinois medical license or be a PA/NP with a Provider Agreement
  • Carry malpractice insurance and provide a Certificate of Insurance (COI) for SWC
Complete the Application
and Rental Agreement
  • Obtain the application form from SWC or their representative
Attend Orientation
and Device Training
  • Upon application review, participation in an orientation program is required
  • Attend training sessions on the center’s technologies
  • Complete training successfully and receive check-off approval for device handling competence
Patients are considered the responsibility of the Provider (member) rather than SWC (presumably a healthcare organization or clinic).
Appointments are scheduled in Nextech, specifically under the outside provider column, indicating that the scheduling system separates these providers from SWC.
Treatments performed on patients are documented in Nextech and sent to the respective provider, similar to how surgical notes are dictated at a surgery center.
Payment for services rendered is collected at the desk using the Provider’s direct merchant processing account. This account is set up during onboarding if necessary, ensuring there is no co-mingling of funds.
The Provider receives an end-of-day report detailing the day’s activities, likely including information about appointments, treatments, and financial transactions.
